Community Partners
In partnership with SCAN, we offer Community Partners services as support for families with children ages 17 or younger living in the home. We can also serve families when they are prenatal or if they have an 18 year old still in school. This program, administered by SCAN, funded by the Indiana Department of Child and Family Services, provides assessments and home-based case management services to connect families to resources to strengthen the family. Families work with their case managers on mutually established goals to move the individuals and family unit towards economic and family stability. Through this program, families can access services such as:

Who is Community Partners for?
Community Partners is designed for families with a child newborn up to 18 years old and not currently enrolled in Healthy Families. To be referred to Community Partners, a family must be referred through this website.
What can families expect?
This program helps families become self-sufficient by utilizing the resources available in the community and ensuring that families have the support they need to be successful. These resources could include, but are not limited to hospitals, faith-based organizations, schools, and First Steps. The staff will identify the resources, make the referrals, and assist families in accomplishing their goals by working together.
